How to Find a Copy of a Personal Identification Card in Pennsylvania
- 1). Obtain a copy of application form 54-B at your nearest branch of the Pennsylvania Department of Transportation (PennDOT) or print the form directly from the PennDOT website.
- 2). Fill out your application and have it notarized. Do not sign the application until you are in the presence of the notary. You wil have to provide the notary with other documents to prove your identity, such as a birth certificate, passport, or Social Security card. Check with him in advance so you can bring what is required.
- 3). Mail the application along with a check or money order for $12 to PennDOT, P.O. Box 68272, Harrisburg, PA 17106-8272.
- 4). Inspect your new photo identification as soon as it arrives by mail. Check for accuracy and the presence of your photo. If your photo is not on record, you will receive a camera card to take with you to a photo center (driver's license center) where a new photo can be taken.
- 1). Locate your nearest Driver's License Center by visiting the PennDOT Locations Info Center site. Enter your zip code in the box provided and check off the box next to "Photo ID Card Duplicate/Replacement." Click the "Search" button at the bottom of the page.
- 2). Visit the location, request a DL-54B application, and fill it out.
- 3). Submit your application along with any forms of acceptable information, such as a birth certificate, passport, or Social Security card. Check in advance so you will know what is required.
- 4). When you have your new card, check it for accuracy as in Step 4, Section 1 (above).
