Download Treasury Form 4000 ("Request to Reissue United States Savings Bonds") from the Treasurydirect.gov website. It can be found by selecting the Forms tab in the top horizontal menu.

Complete the form on your computer following the directions on page 5 of the form. The interactive form 4000 allows you to type in the required information before printing. Do not sign the form after it is printed.

Take the form to your local bank or stockbroker for signature certification by an authorized official. Banks and brokers have managers who can witness and certify signatures in the manner required by the U.S. Treasury. All individuals involved in the name change must have their signatures certified and present photo identification.

Mail the bonds with the completed and signature-verified form 4000 to a Treasury Retail Securities Site. The addresses for the sites are listed on page 7 of Form 4000.
