How to Qualify for Welfare in Ohio
- 1). Acquire a Request for Cash, Food, and Medical Assistance application form. This form is available for download on the Department of Job and Family Services website (see Resources). Alternatively, visit your local County Department of Job and Family Services and ask for a form (see Resources).
- 2). Collect personal and financial information from each person in your household. The application requires monthly income data. You must also list any financial assets held by members of your household, including cash, bank accounts and stocks and bonds. This information is required even if your cohabitants are not applying for welfare benefits themselves. You will be required to verify this information with documented proof.
- 3). Calculate the monthly expense information required by the application. This includes rent and utility payments, child support and daycare costs and medical expenses for other members of your household. Again, include information for everyone living in the home. Documentation for these costs will be required.
- 4). Complete the application form. Take the form to your local CDJFS office. A representative at the office will schedule an interview time for you, during which you will meet with a caseworker to complete the application process.
- 5). Go to your interview at the scheduled time. Bring documentation that verifies the information in your application. Following the interview, you will be informed what assistance you are eligible for, if any.
