Definition of an Attestation Statement
- Different reasons exist for using attestation statements. For example, they may certify that someone has completed a type of training or level of training, confirm quality of job performance or confirm that something was sent or transmitted.
- Sometimes attestation statements verify the information on an insurance claim. For instance, some health insurance companies require physicians to sign attestation statements to confirm a diagnosis and therapy, according to TriWest Healthcare Alliance.
- The Internal Revenue Service stipulates that any summons it sends out must contain a statement of attestation with an official signature. The statement should read, "I hereby certify that I have examined and compared this copy of the summons with the original and that it is a true and correct copy of the original".
