How to Obtain a Disabled Parking Permit in North Carolina

1Review the definition of disabled or handicapped person under North Carolina law section G. S. 20-37-5 to determine if you meet the conditions to which your physician must certify. Decide which form of disability permit you are eligible to obtain based on the nature of your disability.

Download and print the appropriate form which is applicable to the type of disability permit that you are selecting and qualify to receive. If your disability is temporary, download form MVR-37T to request a temporary placard. If your disability is permanent, download form MVR-37A for the permanent disability placard or form MVR-37 for the disability plates. Complete the required applicant information on these forms.

Give the application form to your physician for certification that your disability meets the North Carolina requirements for impaired mobility. The form that you provide to your doctor should correspond to the nature of your disability, either temporary or permanent. Ensure that the proper boxes are checked by your physician and the form is complete so processing is not delayed.

Mail the completed application form to the North Carolina Division of Motor Vehicles at the address shown on the form. Enclose the applicable fees.
