How to Become a Certified SHIP Medicare Counselor in Arizona
- 1). Call the central State Health Insurance Assistance Program office at 602-542-4446 to receive an application by mail.
- 2). Download the SHIP volunteer application on the Arizona Department of Economic Security website.
- 3). Complete the application. Be sure to indicate the hours you're available to work. The shifts offered are from 8:00 a.m. to 12:00 p.m. and from 12:00 p.m. to 4:00 p.m. Monday through Friday. You can choose as many days as you wish.
- 4). Provide an emergency contact and three personal references.
- 5). Indicate whether you're available to attend a four-day training session before you begin working as a SHIP counselor. You must also attend in-service meetings occasionally.
- 6). Fax the completed application to 602-542-6575. You don't have to pay a fee to apply.
- 7). Mail the completed application to Arizona State Health Insurance Assistance Program/SMP, 1789 West Jefferson Street, Site Code 950A, Phoenix, AZ 85007.
- 8). Call 7-1-1 if you need TTY/TDD services to apply.
