How to Apply for Medical Transportation
- 1). Call your local Medicaid office to set up your ride. Give the representative your name, Medicaid identification number and the date and time of your appointment.
- 2). Answer the representative's questions about your transportation needs. Provide the address and phone number of your intended destination and tell her if a family member or aide will accompany you to your appointment.
- 3). Find out if the driver will wait while you're at your appointment or if you'll need to call for pickup afterward. In addition, let the representative know if you'll need to fill prescriptions following your doctor's visit. Based on your needs, the representative will determine eligibility and select a mode of transportation.
