Changes in the size, shape, or position of the breasts can lead to self consciousness and feeling less attractive.  There are many factors involved in breast changes over time.  Many women start with breast shapes upon which they would like to improve, or drooping or sagging breasts that have been present since puberty.  More often, the breasts will develop ptosis (breast droop or sagging) and deflation as a result of changes after pregnancy, with or without breast feeding.  This is also known as post-partum glandular hypomastia with ptosis.  Many women will experience breast deflation and drooping after weight loss, and there are those that develop sagging as a result of time and gravity.

Breast Lift Surgery

Breast lift or mastopexy surgery can raise the position of, and re-shape, your breasts.  Breast lift can be performed as an isolated procedure, but can also be combined with breast augmentation with breast implants (augmentation mastopexy), or with breast reduction (reduction mammoplasty / mastopexy) as indicated.  As a breast lift tightens the breast skin as it lifts the breasts to a perkier position, it may create an "illusion" of a smaller breast, even when no breast tissue is removed.  Hence, many women with breast droop and breast deflation will consider a breast augmentation with saline implants or silicone implants in conjunction with a breast lift.  For women with large, pendulous breasts, breast reduction can be performed with a breast lift to make the large, ptotic breasts more youthfully shaped and positioned.

As a breast lift involves making the "skin bra" smaller, inconspicuous "seams" must be created as the breast skin is reduced and the gland lifted.  A circumareolar mastopexy, or donut mastopexy, is performed with an incision around the areola (dark pigmented skin of the breast surrounding the nipple).  This breast lift procedure, with the incisions limited to the areola, is appropriate when the degree of droop or ptosis is mild, or where some of the excess skin is "taken up" with the placement of breast implants.

As the degree of breast drooping increases, so does the need for additional incisions.  Combining the circumareolar mastopexy or donut breast lift incision with a limited vertical incision is known as a vertical mastopexy, short scar mastopexy, short scar breast lift, or breast lift with a lollipop incision.  When possible, all efforts are made to limit the breast lift incisions to the minimum.  In mild or moderate breast droop or ptosis, particularly when selecting restoration of volume with breast implants, a circumareolar or vertical mastopexy can be performed.

In cases of more severe sagging of the breasts, an incision may be required in the crease beneath the breast.

The breast lift procedure not only restores youthful shape and position, but can also successfully reduce the size and diameter of the areola (if this, too, has become enlarged due to pregnancy, lactation, or weight gain).

Many women who have lost breast volume after weight loss or pregnancy are uncertain as to whether or not they require a breast lift, or if a breast augmentation with breast implants will be sufficient.  When the nipple position is well above the fold beneath the breasts, it is likely that breast enlargement with breast implants will be sufficient.  When the nipple has descended, or when there is any abnormality of shape or asymmetry, breast lift surgery, with or without breast implants, should be considered.
