"It's the End of the World" (season 2, episode 16) When a patient embedded with explosive enters the hospital, it is only the hand of a paramedic in the wound keeping everyone alive.
Beforehand, Bailey returns to the hospital, but only because she is in labor.
However, her husband is severely injured while trying to get to the hospital and is sent to the operating room.
At the very end of the episode, the paramedic with her hand in the wound finally breaks down and runs while Meredith takes over keeping everyone alive with putting her own hand into the wound.
With such a cliffhanger, many fans could not wait to see the next episode.

It sees the birth of Bailey's baby while her husband goes into cardiac arrest (but ultimately survives) and the removal of the explosive device from the patient by Meredith.
Unfortunately, the bomb goes off and kills two of the bomb squad members, although everyone else (including the patient) survives.

"Sanctuary" (season 6, episode 23) In "Sanctuary", the characters are faced with a deadly shooter (named Mr.
Clark) looking for Derek, blaming him for the death of his wife.
