1

Check the definition of allowable disability according to Arkansas rules and regulations to determine if you qualify as disabled before applying for disability parking placards and/or disability plates.

Print the application Form 10-336 from the Arkansas Office of Motor Vehicles website. This form is the Licensed Physician Certification required for issuance of a special license plate or special certificate for the disabled.
3

Take Form 10-336 to your physician so that the certification of the disability can be completed. The physician must indicate whether the disability is temporary or permanent and also indicate the nature of the disability.
