How to Replace a Birth Certificate in Minnesota
- 1). Log on to the Minnesota Department of Health website. Click on either "Certificates and Records" or "Birth Certificate" near the top of the site. Download the Birth Certificate request form.
- 2). Fill out the form completely and truthfully. After entering in all information requested, calculate how much you will need to pay. One copy is $26 through standard delivery.
- 3). Pay by credit card, check or money order payable to the Minnesota Department of Health. Have the form notarized by a notary public. Notaries are often found at banks, insurance offices and law offices.
- 4). Mail the form to the Minnesota Department of Health
