How to Fill Out a Financial Aid Resource & Certification Form

    • 1). Write your legal name, address or other contact information at the top of the form in the designated spaces. Provide your Social Security number or student ID number as requested since the school may not be able to accurately identify you otherwise.

    • 2). List any scholarships or other academic resources you will be using to fund your education in the "Student Resources" portion of the form. Additionally, mention any military, veterans' or other government assistance programs you are receiving funds from, such as housing or vocational training assistance. Do not include federal student loans or federal education grants (i.e., Pell) as resources.

    • 3). Note whether or not you will be attending any other colleges at the same time as the one for which you are filling out the Resource & Certification forms. List the name and location of these schools and if you are currently receiving financial aid there.

    • 4). Read the "Certifications" section of the form. Usually, this includes several statements, such as acknowledging the information provided is accurate, authorizing the financial aid department to apply funds received to any balances you owe, and understanding the requirements for receiving financial aid. If required, initial by each statement.

    • 5). Review the entire form for completeness and accuracy, and make changes if necessary. Sign and date the form before submitting it to the financial aid department at your school.
