Form 1040 Estimated Tax Instructions
- 1). Obtain form 1040-ES from the Internal Revenue Service website. It is located under the "Forms and Publications" link on the main page of their site. You can print the forms yourself or you can contact the IRS to request forms be mailed to you.
- 2). Enter the required information on form 1040-ES. Your name and Social Security number are required as well as your address. If you are a joint filer you must provide your spouse's name and Social Security number as well.
- 3). Enter the amount of your estimated tax payment into the appropriate box. Locate the mailing address for form 1040-ES on the instructions for the form. IRS addresses are divided by state, so locate the state of your residence and mail your voucher and check, payable to the U.S. Treasury to the correct address.
- 4). Mail all of your quarterly estimated taxes on time. They must be postmarked by the deadline for each quarter. Your first quarter payment is due April 15, second is due June 15, third is due Sept. 15 and the final payment is due Jan. 15.
