    Applicants Holding a CSW

    • 1). Complete at least 4,000 hours of clinical social work supervised by an LCSW licensed for at least two years. The 4,000 hours must include 1,000 hours of practice in mental health therapy, at least 100 hours of which you must perform under direct supervision.

    • 2). Download the social worker application from the Utah Division of Occupational and Professional Licensing, or DOPL, website. The application file includes forms that you may need to submit with your application.

    • 3). Complete the portions of the application that pertain to becoming an LCSW. Be sure to follow the instructions in the application file.

    • 4). Ask your supervisor to complete the Verification of Clinical Social Work and Mental Health Therapy for Licensure as a Licensed Clinical Social Worker form.

    • 5). Skip Steps 6 and 7 if you passed the ASWB clinical-level exam in another state. Instead, ask that state to complete the Request for Verification of License form and mail it to the DOPL.

    • 6). Register for the ASWB clinical-level exam after you receive a letter from the DOPL stating that you meet the requirements for pre-approval to take the exam. The letter will explain how to register for the exam.

    • 7). Take the ASWB clinical-level exam. The testing agency will send you an unofficial score report that documents your passing score.

    • 8). Submit the application, all required documentation and fees to the DOPL by regular mail, express mail or in person to the address shown in the application. If Utah issued your CSW license, it is not necessary to submit a transcript because you submitted it when you applied for your CSW license. Fees are non-refundable. It usually takes from 10 to 15 working days for the DOPL to process an application.

    Applicants Holding an LCSW Issued by Another State

    • 1). Download the social worker application from the DOPL website. The application file includes forms that you may need to submit with your application.

    • 2). Complete the portions of the application that pertain to becoming an LCSW. Be sure to follow the instructions in the application file.

    • 3). Obtain verification of at least three years of licensure from the state where you hold an LCSW license. Use the Request for Verification form.

    • 4). Submit a Verification of Active Practice as a Licensed LCSW form. It should document that you have practiced as an LCSW for at least 4,000 hours, of which 1,000 hours were in mental health therapy.

    • 5). Submit the application, all required documentation and fees to the DOPL by regular mail, express mail or in person to the address shown in the application file. Fees are non-refundable. It usually takes from 10 to 15 working days for the DOPL to process an application.
