Submit a letter to your local IRS office in Texas. The letter should include a statement of the suspected instance of tax fraud along with the name and the most recent address of the suspected violator. The taxpayer identification number is also important to include in the letter. For an individual, the taxpayer identification number would be a Social Security number, and a business would have a federal employer identification number (FEIN). Access the IRS.gov website (see Resources) and print out form 3949-A. Fill out the form entirely, following the included directions. Then forward the form by mail to the Internal Revenue Service in Fresno, CA 93888.
