    • 1). Click the "How Do I..." tab on the US Department of Interior Office of Indian Affairs website. Follow associated links for establishing lineage or contacting tribal offices. Contact the USI to obtain a heritage registration form, at: 202-208-3710.

    • 2). Request a CDIB (Certificate of Degree of Indian Blood) Card application and/or heritage application form from the relevant tribal agency. Applications and forms are found, for example, on the official Chickasaw and Cherokee Nation web pages -- or your tribal agency's website. Fully complete applications and forms -- as necessary -- and sign any attached sworn affidavit statements. Be prepared to provide copies of your official birth certificate, a parent's birth or death certificate, blood test results and/or copies of court orders or other documentation validating your claim.

    • 3). Submit your application and/or forms -- and all necessary documentation -- to the tribal agency via postal mail. Follow up with your tribal agency 14 days after submission to confirm your registration status. Proceed to Step 3 as an additional measure for heritage registration.
