    • 1). Contact the probate court in the Michigan county court located where the decedent lived. Ask the court clerk to provide you with Probate Code Form 558 (PC Form 558). Alternatively, download and print this form (see Resources).

    • 2). Fill out PC Form 558. In this form, you must provide your name, your relation to the decedent and information about the decedent and his estate. On Line 9 of the form, write in your name as the nomination for personal representative and check box No. 15 asking the court to appoint you as the personal representative.

    • 3). Send this form to the court clerk. Send copies of this form to other interested parties including creditors and heirs. If there are no objections to the appointment, the court will provide you with a completed copy of PC Form 572, the "Letters of Authority." This document enables you to act as personal representative of the estate.
