A 38-Year-Old Woman With Anisocoria

Clinical Presentation


A 38-year-old woman noted that her left pupil was larger than the right pupil after exercising in the gym 3 days prior to her eye examination.

There is no family history of vision loss.

She does not drink alcohol or smoke tobacco. She is taking no prescription medications and only occasionally takes ibuprofen.

Visual acuity was 20/20 for each eye. Color vision with Ishihara pseudoisochromatic plates was 10/10 for each eye. Anisocoria was noted, with the left pupil larger than the right. The anisocoria was greater in the dark, and both pupils were briskly reactive to light. Slit lamp examination of the anterior segment was normal for both eyes. Funduscopic examination also was normal for both eyes. Ductions and versions were normal, and no evidence of an ocular misalignment was found.
