How to Get a Copy of My Birth Certificate in Hamilton County, Ohio
- 1). Check the name of the place where you were born. If you were not born at Bethesda North, Jewish Kenwood, Our Lady of Mercy - Anderson, Our Lady of Mercy - Mariemont, or Maple Knoll hospitals, you must check with the Cincinnati, Norwood, or Ohio Health Departments. If you were born at one of these hospitals, proceed to step 2. If you were not, proceed to step 5.
- 2). Download and print the Application for Certified Copies of Birth and Death Records from the Hamilton County Public Health website. It is available as a Portable Document Format (PDF) file.
- 3). Complete the application and submit it along with payment. You can do this in person or via postal mail.
