How to Correct the Date of Birth after a Naturalization Oath
- 1). Download Form N-565, Application for Replacement Naturalization/Citizenship Document, from the U.S. Citizenship and Immigration Services website (USCIS). Fill out form N-565 with the relevant information including your name, address, correct date of birth and telephone number.
- 2). Check box "b" for "New Certificate of Naturalization" in response to "Type of application." Then, check box "d" for "My certificate or declaration is incorrect" in response to "Basis of application. In Part 3, indicate your gender, height and marital status. Write down the issuing agency and date of your naturalization certificate.
- 3). Write an explanation in Part 5 of the form as to what is incorrect and why it is incorrect. Explain that your birth date is incorrect and why you believe it to be incorrect. Attach to the form a copy of your birth certificate or other identifying document with the correct birth date. Sign and date form N-565.
- 4). Obtain a photograph of yourself, taken within 30 days of the filing of form N-565. The photo must be in color with a white background. Generally, these requirements are the same for passport photos. Use a business offering passport photos to obtain a photo with the correct requirements. Attach two identical photos to form N-565.
- 5). Mail your documents to the USCIS at the address indicated in the instructions. Note that the address is different depending on your state of residence. No filing fee is required for correcting information on your certificate. Await processing and receive your new naturalization certificate in the mail.
