How to Replace a Wisconsin Birth Certificate
- 1). Go to the Wisconsin Vital Records Department at 1 West Wilson Street in Madison, Wisconsin. The Vital Records office is in Room 158. They are open from 8 a.m. to 4:15 p.m. Monday through Friday. Be sure to bring your driver's license or state issued identification card that has your current address on it. You can get your birth certificate by 10 a.m. the next business day if you apply by 2 p.m. The cost is $20 as of 2010, which can be paid by cash, check, or money order. If you need your certificate within 30 minutes, the fee is $40 for expedited service. The application for a birth certificate is listed below under Resources.

Enclose a check or money order for $20 payable to Wisconsin Vital Records, and a self-addressed, postage paid, business-size envelope. You will receive your certificate in approximately one month. Expedited service is not available by mail. You do not need to include identification with your application. - 3). Fax your application to 608-255-2035. Use the fax application that is listed under Resources. All faxed applications cost $40 to process and are mailed out within five business days. Put your credit card number on the application for the birth certificate fee. You also can choose overnight shipping for your certificate by UPS for an additional cost of $17.50.
