• 1). Visit www.vitalchek.com/express-death-certificates.aspx to begin the application process. Vital Check is the third-party provider for processing certificate requests for the State of Utah.
  
  
• 2). Select the type of certificate you wish to order and click "Continue."
  
  
• 3). Specify the city in which the death occurred. Click "Continue."
  
  
• 4). Enter the date the death occurred. Click "Continue."
  
  
• 5). Select the reason for the death certificate request. Click "Continue."
  
  
• 6). Select the certificate you wish to purchase. Click "Continue."
  
  
• 7). Confirm that you are authorized to order the death certificate. Using your credit card billing information, VitalChek will confirm your identity and relationship to the deceased. A representative may need to contact you for relationship confirmation prior to releasing the certificate. Click "Continue."
  
  
• 8). Enter the amount, if any, of additional certificates you wish to order. There is a fee for each copy. Click "Continue."
  
  
• 9). Enter your current legal name and your relationship to the deceased. Click "Continue."

Order by Phone

• 1). Download and complete the death Certificate Request Form from silver.health.utah.gov/applications/Death.pdf to ensure you have all the required information.
  
  
• 2). Call toll-free 1-866-632-2602 during regular business hours on Monday to Friday. These orders are processed by the same company as the online orders, Vital Check. Credit card payment is the only option for those ordering a death certificate.
  
  
• 3). Provide the operator with the required information about you, the deceased and your payment and shipping options.
  
  

Order by Mail

• 1). Download and complete the Death Certificate Request Form from silver.health.utah.gov/applications/Death.pdf and ensure you have all the required information.
  
  
• 2). Determine how many certificates you need to order. The fee is $16.00 for the first copy, and $8.00 for each additional copy.
  
  
• 3). Get a scan or photocopy of your identification that is enlarged and easy to read.
  
  
• 4). Mail a check or money order for the appropriate sum, the completed form and your copied identification to:
