Diverticular Disease: Presentation and Management

Conclusion

Much of our previous understanding of diverticular disease has recently been challenged. The pathogenesis of diverticular disease is likely multifactorial and more complex than previously believed. Currently, the role of dietary fiber and colonic motility is being re-evaluated, but we would not yet recommend against increased dietary fiber intake, which likely has net health benefit in general. Although the role of mesalamine was initially promising, the most recent and larger studies failed to demonstrate benefit. As our understanding of SUDD continues to advance, new medications and treatments will likely be added to our armamentarium. Meanwhile, our current management practices for treating uncomplicated diverticular disease are likely to involve fewer surgical interventions and perhaps more judicious antibiotic use.
