How to Get a Copy of Your Birth Certificate in California
- 1). Download and complete an "Application for Certified Copy of Birth Record." The application requires the full name of the person named on the record, the name of the person ordering the record and her relationship to the person named, the date of birth, city of birth and parents' names of the person named. You must also include your mailing address.
- 2). Get the application notarized if you are ordering an authorized copy. The application contains a "Sworn Statement" section. The person ordering the record must get their signature verified by a notary.
- 3). Write a check or money order out to the "Office of Vital Records" in the amount of $14 for either an authorized/certified copy or informational copy.
- 4). Mail completed application and payment to:
