How to Obtain a Copy of a Tax ID Form
- 1). To obtain a paper copy of the SS-4 (Tax ID) form, call (800) TAX-FORM, or (800) 829-3676.
- 2). Request to have an SS-4 form mailed to you. The instructions for completing the form should be included.
- 3). If you prefer not to call the 800 number, a second option is to visit the IRS website at www.irs.gov. All tax forms are downloadable in PDF format (see Resources).
- 4). A third option is to complete an online order form on the IRS website. On the order form you may request various tax forms and corresponding instructions. Once you finalize your order the forms will be mailed to the address you specify. This order form allows you to request multiple copies of each form and instructions (see Resources).
