    Birth Certificates Dated Before 1990

    • 1). Apply in person at:
      1818 W. Adams St
      Phoenix, AZ 85007

      Bring a government issued picture identification card and the fee. The state accepts the following payments: money orders, cashier's checks, traveler's checks, cash, Visa or MasterCard. Make your payment payable to the Office of Vital Records.

    • 2). Complete the Request for Copy of Birth Certificate. This form is available on the Arizona Department of Health Services website. Attach a copy of your government issued picture identification card and the fee. As of June 2010, the fee is $10. Mail your request to:

    • 3). Contact VitalChek to use their online services. VitalChek has partnered with the state of Arizona to process online requests for birth certificates from qualified applicants. A convenience fee is charged for using VitalChek's services. VitalChek accepts all major credit cards and ships using expedited service or UPS delivery. An additional shipping fee is charged for shipping. Contact VitalChek on their website or by phone at 877-803-0609.

    Birth Certificates Dated After 1990

    • 1). Complete the Vital Records Request for Copy of Birth Certificate form. This form is available on the Yavapai County website or the Vital Records Office. You need to provide the personal identifying information of the certificate holder, the applicant's personal identifying information, the purpose for your request and your contact information.

    • 2). Pay the fee. As of June 2010, the fee is $10. Yavapai County accepts cash, cashier's checks, money orders and personal checks. Make your payment payable to the Yavapai County Community Health Services (YCCHS).

    • 3). Attach a copy of a government-issued picture identification card. Alternatively, you can have your application notarized by an Arizona Notary Public.

    • 4). Submit your request via the lock box in the Vital Records Office lobby or mail your application to:

    • 5). Wait seven to 10 days for your certified copy to be processed. The County mails your certified copy to you. Pick up service is not an option.
