Driver's License

• To change your last name on your California driver's license or identification card you must visit California Department of Motor Vehicle location and complete a Driver License or Identification Card Application form DL44 or DL 44C form. You must have your Social Security card changed at the time of the driver's license or identification card name change so that the information can be verified. The Department of Motor Vehicles electronically verifies Social Security information so you do not have to have a Social Security card reflecting the name change for the information to be confirmed. You will be required to submit a thumb print and have your picture retaken for your new license or identification card.
  
  

Social Security Card

• Changing your California Social Security card requires that you provide documents that show your legal name change, your identity and your citizenship status. Your marriage license will serve as the document displaying the legality of your name change and your driver's license or identification card will prove your identity. Your citizenship status can be proven through your U.S. birth certificate or your immigration documents. You must complete the Application for a Social Security Card Form SS-5 and submit the documents and the application by mail or in person to the Social Security Office in your area. Your Social Security number will not be changed; only the name will be changed on your new card. All documents submitted must be originals and any documents mailed will be returned to you by mail.
  
  

Fees

• The cost for completing a name change for your California identification card was $26 as of February 2011. Individuals 62 years of age or older are not required to pay a fee for the name change on their identification card. Reduced fee identification cards are provided to individuals who meet income requirements from a public assistance program. The fee to change your name on a California Driver License was $25 as of February 2011. There is no fee for changing your Social Security card information.
  
  

Replacement Timeline

• You will be provided with a temporary license or identification card with your new name information that is valid for 60 days from the day you complete the name change. This form will be a paper document and will not have a picture on it. You will be able to keep your photo license or identification card until your new document is delivered in the mail. Your replacement license or identification card may take six to eight weeks to be delivered by mail. Your replacement Social Security card will be mailed to the address listed on your provided identification within seven to 14 days.
