How to Send a Recreated W-2 to the IRS
- 1). Request your W-2 Form from your employer or payer if you have not received it by Jan. 31, or if there are mistakes on your W-2. Give your employer until Feb. 14 to send you the correct W-2 Form.
- 2). Call the IRS at 800-829-1040 for assistance if you have not received your W-2 Form by Feb. 14. The IRS will ask for your name, address, phone number, Social Security number and employment dates. Additionally, you will need to provide your employer's name, address and phone number. The IRS will contact your employer and request that they send you your W-2 Form. They will also send you a Form 4852, which is a substitute for Form W-2 or Form 1099-R.
- 3). Fill out Form 4852. Start with your name, Social Security number and address (lines one through three). On line four, verify that you were unable to get a correct W-2 from your employer and that you've notified the IRS of that fact. Enter the employer's name and address on line five.
- 4). Enter the employer's tax identification number (TIN) on line six. The TIN may be printed on your pay stub. If you have filed a tax return using this company in previous years, find the TIN number on your previous W-2. If you cannot locate the TIN, leave the field blank. Fill out line seven using your final pay stub for the tax year. Skip line eight. On lines nine and ten, explain where you got your numbers (from your final paystub for the tax year). Also, detail the efforts you made to get the correct Form W-2 from your employer. Sign and date the form. Make a copy or scan of the form for your records and send it in to the IRS with your income tax return.
