    • 2). Download IRS Form 4506-T or 4506-T-EZ on the IRS website. The IRS provides these forms for free. Alternatively, you can request a copy of the form by mail or by phone.

    • 3). Complete Form 4506-T by providing your name and Social Security number or tax identification number. If you filed your taxes jointly, you must enter the primary taxpayer's name first. Provide your address in line 4.

    • 4). Enter the specific forms you are requesting in line 6. To obtain information regarding your previous year's adjusted gross income, enter Form 1040 or 1040NR, EZ, or 1040A. If you did not file a tax return, you must include this information in line 7, and the IRS will assess penalties and interest for failure to file.

    • 5). Enter the previous tax year for which you are requesting tax returns. You may request up to four years of tax returns on one Form 4506-T. If you need more than four years of tax returns, you must complete an additional tax request form. As of 2011, the IRS does not charge taxpayers for copies of their tax returns.

    • 6). Sign and date the bottom of Form 4506-T. If you filed joint tax returns, your spouse must also sign the form. You can fax or mail it to the IRS processing center that serves your area. You may locate addresses and fax numbers on the official instructions for the form.
