    • 1). Open your Internet browser and navigate to the Apple service and Repair shop (link provided in "Resources").

    • 2). Type in the serial number (located on the back of the iPhone) into the "Hardware serial number" bar in the middle of the Service and Repair site.

    • 3). Select a location nearest to you and create a service report. This report informs the individuals at the Apple repair facility what is wrong with your iPhone (the lens). It also allows the individuals to look up and see if your device is still on warranty. If not you must pay for the service up front). Once the service report is created your nearby service location receives the information.

    • 4). Fill out the payment information (if required) and submit the payment. This information includes your mailing address, billing address and telephone number.

    • 5). Mail the iPhone to the service address provided for you (the locations are going to vary depending on where you live). In a few business days (typically around three) you receive your iPhone back, with the replaced lens.
