How to Find Out If I Am Due Any Military Medals

    • 1). Download the Standard Form 180 from the National Archives website (see Tesources). Complete Section 1 of the form as thoroughly as possible. In section 2, mark "Other" as what you would like a copy of and write "medals due" in the space provided. For the reason for the request, mark "medals/awards." Complete section 3, which must include your signature.

    • 2). Mail the form for Army, Army Air Corps and Air Force medals, to:

      National Personnel Records Center

      9700 Page Ave.

      St. Louis, MO 63132-5100

      Medals are mailed from a different location after the National Personnel Records Center verifies service and awards due. See References for addresses for Army, Air Force and Army Air Corps award mailings.

    • 3). Mail the form for Navy, Marine Corps and Coast Guard medals to:

      Navy Personnel Command

      Liaison Office Room 5409

      9700 Page Ave.

      St. Louis, MO 63132-5100

    • 4). Request a search through eVetRecs (see resources). If you are a military veteran of the Army or Air Force, or the next of kin of a deceased Army or Air Force veteran, you may request records and medals online. EVetRecs does not allow Navy, Marine Corps or Coast Guard veterans to place a request online for medals.

      Complete the forms in eVetRecs as indicated. You will need to print and sign the request form. It will be submitted electronically, but a faxed or mailed copy bearing your signature must be sent. All requests are processed through the National Personnel Records Center.
