    • 1). Log onto the official Social Security Administration website and download form SS-5 (Application For a Social Security Card). The application location is listed in the reference section.

    • 2). Complete the form in ink, providing your identity and citizenship information. You'll need both parents' Social Security numbers as well as your own to complete the form.

    • 3). Take or mail the form along with original or certified documents proving your identity and citizenship status to the Social Security office for your area. Locate your local Social Security office by performing a ZIP code search on the Social Security Administration's website. The search form is listed in references. Widely accepted identity documents include state-issued driver's licenses or ID cards and U.S. passports. Acceptable citizenship papers include: U.S. birth certificate, U.S. passport or Certificate of Naturalization or Citizenship.

    • 4). Wait for the Social Security Administration to verify your documentation before sending your new card in about three weeks.
