How to Apply for Oregon Welfare Benefits
- 1). Get a copy of the application for welfare services in Oregon (DHS 0415F: Application for Services). Pick one up at your local self-sufficiency office or print one from the Department of Human Services website (see References).
- 2). Fill out the application for Oregon welfare services. Depending on what type of benefits you want, there are different parts of the form for you to fill out. All welfare benefits required you to fill out the first four pages and sign page 14. If you want food benefits, you must fill out page five. If you're hoping for cash assistance or medical benefits, you have to fill out the entire document. The application basically tries to gauge your need level and what type of assets you have so it can make sure the state's limited benefits go to those most in need.
- 3). Find the self-sufficiency office closest to you (see Resources). There are DHS offices in each county; be sure to look for the self-sufficiency office.
- 4). Turn in your application for Oregon welfare services. Be sure to make a copy for your records.
- 5). Make an appointment with a caseworker. According to the Department of Human Services, a social worker often must meet with you in order to approve welfare benefits for you and your family.
