Go to your local civil court and request an application for a pro se summons form, or download and print the summons form from your local civil court's website. Complete the form in its entirety to prevent any processing delays from occurring, and return the form to the civil court in the county in which you will be filing the case against the defendant. - 2

Ask the clerk of court what the fees are for filing a summons, or visit the civil court's website for a list of the court fees. Pay the appropriate fees to the clerk of court when you submit the summons application. Pay your fee with cash, a money order or a bank or certified check, as certain courts do not accept credit cards and personal checks. Serve a copy of the summons to the defendant, or request that the summons be served upon the defendant by a court official. - 3). File the original summons with the court, along with proof that you served the defendant the summons (such as an affidavit of service), to continue with the filing of your case. Pay the filing fee for the case, so that the summons and other necessary documents can be given to the judge assigned to your case for review.
