How to Apply for Disability in Ohio
- 1). Apply for disability with the Ohio Department of Job and Family Services. The application is on the website (see Resources section). The website also lists the locations of one-stop centers, where people can help with sign language interpretation, filling out registration forms, using a recorder to help remember things, and other tasks. Ohio offers a list of one-stop locations (see Resources section).
- 2). Provide your household's address, your name and your signature on Page 1 of the application and submit this to the state Department of Job and Family Services. This will preserve your filing date. Do this before working on the rest of the application.
- 3). Complete the application. You will need basic information such as your Social Security number, as well as detailed information involving your income and expenses. You must be able to prove all the information you provide.
- 4). Gather bank statements, pay stubs, and as much documentation as possible to verify the information in your application. Photocopy everything, and keep a copy in your files.
- 5). Schedule an interview with a representative from the Department of Job and Family Services.
