How to Get W-2s and 1099s From Years Passed
- 1). Download and print IRS Form 4506.
- 2). Enter your first and last name in box 1 of the form. Enter your Social Security number in box 1b of the form. If you filed a joint tax return for the tax year you want to request, enter your spouse's name in box 2 and your spouse's Social Security number in box 2b.
- 3). Enter your current address in box 3 of the form, and enter the address used on the tax return you want to request in box 4.
- 4). Enter "1040" in box 6.
- 5). Enter the tax years requested in box 7. You can request up to eight tax years using a single form.
- 6). Enter the number of years requested in box 8b, and multiply this number by $57. Enter the total in box 8c.
- 7). Sign and date the Form 4506. If requesting a joint tax return, your spouse will also need to sign the form.
- 8). Mail the completed form and payment for the amount listed in box 8c to the address listed on Page 2 of the Form 4506. The address you send the form to depends on the address listed in box 4 of this form.
