How to Change Your Name in New York State for Social Security
- 1). Complete form SS-5. You can download and print the form from the Social Security website or you can call your local office and ask them to mail you a copy.
- 2). Prepare the necessary documents. You must have a marriage license, a divorce decree or a court order to change your name. You'll also need a state-issued photo ID or a passport and your birth certificate, passport or resident alien card.
- 3). Submit the application to the Social Security office along with supporting documents. New York state residents can apply in person at a local Social Security office or mail the application and documents to the nearest office. If you live in New York City, you may visit two locations.
