That is the form your ex has to turn in if he is going to participate in the SBP. On the form there is a section called "election of coverage." Under that section he will check who all he wants to be covered by the plan. There is a place he can check for his former spouse. That would be you.

There isn't a place where you need to sign the form. He would do that since he is the one being insured.

You are the beneficiary and there may be a place where your address and other information is needed. You need to look at the form and make sure he has checked that his former spouse is the beneficiary and also look to see if there is a place to input information about the beneficiary.

Get the form and read it from top to bottom and then make a copy of it for your records.
