- 1). Visit the Social Security Online website. The Social Security Online website is the official website of the U.S. Social Security Administration.
- 2). Complete and print the social security card replacement application. The application is also known as Form SS-5. You can find the application by clicking "Social Security Card" under the "Select Below to Apply For" header on the homepage of the Social Security Online website. On the next screen, under the "More Information" header, click "Replacing a Lost Card." A link to the application is located in "Step 1," under the "Replacing a Lost Social Security Card" header.
- 3). Attach original documents with the application proving your daughter's identity and U.S. citizenship. You can prove your daughter's U.S. citizenship with a U.S. birth certificate or U.S. passport. You can prove your daughter's identity with a U.S. passport, school identification card or with doctor, clinic or hospital records.
- 4). Attach original documents proving your identity. You can prove your identity by attaching your U.S. driver's license or U.S passport with the application.
- 5). Take the completed application and all required documents to your local Social Security office. If you don't want to take the application to your local Social Security office, you can mail the completed application and all required documents to your local Social Security office instead. You can find the mailing address for your local Social Security office in your phone book.
- 6). Wait patiently for the Social Security Administration to verify all documents received. According to the Social Security Administration, your child's card will be mailed as soon as the Social Security Administration receives your child's information and has verified the documents provided.
