    • 1). Review the circuit grants qualification to ensure that you qualify for the program. For example, you qualify if you are 65 years or older or at least 16 years old with a disability. You must be an Illinois resident at the time of the application. You also must meet the income limits. Although the income maximums change, single 2011 applications can make no more than $27,610. Spouses or you and a qualified co-resident can make no more than $36,635 combined.

    • 2). Gather documents you need to complete the application. You need proof of income in the form of a W-2, 1099 or other financial documents that prove the amount of income you earned for the year proceeding the year you submit the application. For example, if you are applying in 2011, you need your 2010 income information.

    • 3). Complete either the paper or online applications. Be prepared to disclose, among others, your name, address, spouse's information and an itemization of your income. Also, you must list the property taxes and rent you paid for the year preceding the year in which you are submitting the application.

    • 4). Mail the application by December 31 of the year in which you are applying. Send the documents to one of the addresses listed on page 4 of the application. For example, one address is used if you are only applying for a grant. A separate address is used if you are applying for multiple benefits, such as the property tax grant and the prescription drug benefit.
