    • 1). Call the bank at the customer service phone number provided on your monthly statements or the bank website. Use your keypad to navigate the menu and reach a representative.

    • 2). Give the representative your name, account number and identifying information, such as your date of birth and Social Security number.

    • 3). Ask the representative if you have any holds, garnishments or levies on your checking account. She will examine your account and, if a levy is present, tell you the name of the creditor or government agency that ordered the levy and the date the levy appeared on the account.

    • 4). Request a copy of the levy order. This document contains the case number, the creditor's name, the court in which the lawsuit was filed and the date the court made the judgment. Keep it for your records.
